With the implementation of electronic health records we have streamlined the time involved with completing forms for our new and established patients. There is no need to download, complete, and or send any forms to us prior to your appointment. You will simply confirm your printed unique identifier forms with your signature containing the information you provided to us previously when scheduling your appointment. The HIPAA form has one and up to three lines to complete with your contacts and telephone numbers and will also require your signature. If scheduled for a stress test the consent form will be printed upon your arrival for your review and signature.

When calling to schedule your appointment, please provide the following pertinent registration information:
Date of birth
Brief description of the reason for the appointment
Home Address
Home and alternate phone numbers
Health insurance information
Name of referring physician and or PCP
Emergency Contact and phone numbers
Referral form (have it with you) to ensure scheduling of the appropriate appointment
This information will be entered for you and your insurance information will be verified by our billing specialist prior to your scheduled visit or test.
